New Allied Health Group Services for patients with Type 2 Diabetes
From May 1 2007, patients with Type 2 Diabetes who also have a GP Management Plan in place are eligible to receive an initial assessment and up to 8 group sessions with a diabetes educator, exercise physiologist or dietician in addition to the 5 allied health services under team care arrangements. A separate group services referral form is required to refer patients to a registered allied health practitioner (a diabetes educator, exercise physiologist or dietician) who will conduct an initial assessment. From there on, the allied health practitioner will determine if the patient would benefit from group services and the type of services they need to access.
